1. Nota firmada por el Representante Legal del negocio, solicitando la reconsideración de la multa, dirigida a la Dirección de Administración Tributaria, incluir en la misma, número de teléfono. (1 copia)

2. Certificado del Registro Público o copia autenticada por Notario (Vigencia de 3 meses).

3. Copia de cédula de Representante Legal (no se acepta ampliada, escaneada, ni fotografiada).

4. Certificado de Aviso de operación actualizado del sistema www.panamaemprende.gob.pa

5. Estado de cuenta municipal actualizado.
6. Declaraciones de renta con sus respectivos certificados de recepción de la DGI.

7. Certificación de Contador Público, debe ser firmada original con fecha actualizada, traer copia de cédula, Idoneidad.

EN CASO DE NO SER EL REPRESENTANTE LEGAL DEL NEGOCIO DEBE TENER AUTORIZACIÓN NOTARIADA Y COPIA DE CÉDULA DEL AUTORIZADO.

NOTA: NO SE ACEPTAN DOCUMENTOS ESCANEADOS, FOTOS, NI LAS SOLICITUDES EN MANUSCRITOS.
